【摘要】 目的 观察褪黑素(MLT)对庆大霉素(GM)所致大鼠急性肾损伤的保护作用,并探讨其可能的机制。方法 将24只Wistar大鼠随机分为GM组、GM+MLT组 、对照(Cont)组。应用药物前后,分别记录并观察各组大鼠体质量和24 h尿量的变化;检测并观察用药后第5天血尿素氮(BUN)、肌酐(Cr)、尿N乙酰βD氨基葡萄糖苷酶(NAG)以及血清、组织匀浆中丙二醛(MDA)、过氧化物歧化酶(SOD)、谷胱甘肽过氧化酶(GSHPx)各指标的变化;处死大鼠后经肉眼观察和光镜检查肾组织的形态学变化。结果 3组大鼠的肾组织形态学评分比较,差异有显著性;GM+MLT组血BUN、Cr,尿NAG以及血清、组织匀浆中MDA、SOD、GSHPx各指标与GM组比较差异有显著性(t=-10.41~9.55,P<0.01)。结论 MLT可通过抗氧化作用减轻庆大霉素对肾脏的损伤,对急性肾损伤具有一定的保护作用。

[ABSTRACT]ObjectiveTo study whether melatonin possesses protective effects on gentamicininduced acute renal injury and approach its possible mechanism.MethodsTwentyfour adult Wistar rats were randomly divided into three groups: GM group, GM+MLT group and Control group. The weight and urine volume were recorded before and after the experiment. Blood urea nitrogen (BUN), serum creatinine (Cr), the level of malondialdehyde (MDA) in serum and renal tissue, the activities of superoxide dismutase (SOD) and glutathione peroxidase (GSHPx) in serum and renal tissue were measured before and five days after gentamicin and melatonin were administered. Renal histomorphological changes were observed through naked eyes and microscopically after the mice were sacrificed. ResultsThe difference of histological grading of kidney between the three groups was significant. The levels of BUN, Cr, MDA, SOD and GSHPx between the GM group and GM+MLT group were significantly different (t=-10.41-9.55,P<0.01). ConclusionMelatonin provides certain protection against gentamicininduced acute renal injury. The antioxidative functions of melatonin might be involved in the protective effects.

庆大霉素(GM)作为氨基糖甙类抗生素,具有抗菌谱广、疗效较可靠、价廉等优点,临床广泛应用于革兰阴性菌、肠球菌及分支杆菌感染治疗[1]。有文献报道应用GM治疗后7 d以上有10%~20%病人出现急性肾衰竭[2],增加了治疗风险及治疗成本,极大地限制了GM的应用。因此,对GM肾毒性的机制及防治方法的研究具有重要的临床意义。褪黑素(MLT)是由松果体合成分泌的一种吲哚类神经内分泌激素,具有很强的抗氧化作用。实验研究表明,MLT对毒物或其他因素所致的急性肝、脑损伤有一定的保护作用[3,4]。本实验通过形态学和功能学研究,探讨MLT对GM所致急性肾损伤的保护作用及其可能的机制。
1 材料与方法
1.1 动物及分组
健康成年Wistar大鼠(购于青岛市实验动物中心)24只,雌雄各半,体质量180~220 g。随机分为GM组、GM+MLT组、对照(Cont)组,每组8只,雌雄各半。于实验前1周分别将其单独饲养于金属代谢笼中,给以充分的标准饲料、水及自然光。
1.2 实验方法
GM组:每次将GM(青岛国风制药厂提供,批号H37021973)70 mg/kg皮下注射,每天2次,共4 d;GM+MLT组:GM剂量及注射方法同GM组,在皮下注射GM前给予MLT(由Sigma公司提供)5 mg/kg腹腔注射,每天1次,共4 d;Cont组: 用等量生理盐水代替GM和MLT,分别经皮下和腹腔注射,共4 d。
